Notice on Online Application of Health Declaration Form

Starting from 23 November 2020, all foreign passengers who are flying from the Philippines to China will be required to present the Electronic Health Declaration Form for boarding online. The Chinese Embassy and Consulates in the Philippines will no longer accept applications submitted by email.

Foreign passengers can apply for the Electronic Health Declaration Form by logging on the website https://hrhk.cs.mfa.gov.cn/H5/via computer or smart phone. On this website, foreign passengers should declare their health status and upload negative nucleic acid test and IgM anti-body test results (both tests must be conducted within 48 hours before boarding).

The Chinese Embassy and Consulates in the Philippines will examine and verify the above documents online, and then issue the Health Declaration Code with the "HDC" Mark (sample attached below ) to passengers. Passengers must present the Green Health Declaration Code with the "HDC" Mark within its validity period to the airlines flying to China before boarding.

Please note that the Green Health Declaration Code with the "HDC" Mark is NOT a visa, for information on visas to China, please refer to the Embassy's Notice on the Temporary Suspension of Entry into China by Non-Chinese Nationals in the Philippines Holding Valid Chinese Visas or Residence Permits on 5 November 2020.
